I've been working at Penn Medicine since August of 2019 as an Innovation Project Associate in the Acceleration Lab at the Center for Health Care Innovation.

I've been involved in multiple design projects, with topics ranging from:

  • precision medicine for ovarian cancer patients
  • antibiotics stewardship for pneumonia
  • inpatient psychiatry care
  • primary care clinic task management
  • continued geriatric care after discharge
  • ICU code call documentation

Precision Medicine in Ovarian Cancer

How might we increase percentage of ovarian cancer patient getting the appropriate kind of genetic testings?

Challenge

With the FDA-approved PARP inhibitor drugs, guidelines recommend clinicians offer genetic testing to every ovarian cancer patient. However, not every ovarian cancer patient completes the appropriate kind of genetic testings.

Outcome

This project is still in discovery phase, where we are trying to define the problem statement.

Pneumonia antibiotics stewardship

How might we reduce over-prescribing of antibiotics treating pneumonia in inpatient setting?

Challenge

Antibiotics for pneumonia are often prescribed longer than what guidelines recommend. Although existing decision support tools within the EHR can suggest a proper treatment duration if given the correct indication (e.g. community-acquired pneumonia) and date of first dose, these tools fail in pneumonia cases because patients may transition across multiple antibiotics and the first few doses are sometimes ordered prior to an established diagnosis of pneumonia.

Outcome

A web-based dashboard addressing shortcomings of current EHR decision support tools. It is designed for use by the Antimicrobial Stewardship team to quickly identify overprescribing and to intervene by contacting the primary team to discuss the treatment.

How might we reduce the cognitive load and the amount of time of data-entry and documentation for psychiatry providers?

Challenge

MEND (Mental Health Engagement, Navigation & Delivery) is a solution to identify patients with psychiatric issues upon admission, deliver enhanced care during their hospitalization, and connect patients to ongoing mental health care after discharge.

For the MEND delivery team, there are multiple medium for documenting their care, including a web-based dashboard, paper spreadsheet, and the EHR system. There is often duplication of work regarding data-entry, which adds burden on them from spending more time with patients.

Outcome

In the process of re-designing the documentation and communication system that combines the web-based dashboard, paper spreadsheet, and PennChart (EHR system).

How might we help primary care providers ensure that their patients are completing their referrals?

Challenge

With the high volume of patient, it is hard for primary care providers to keep track of whether their patients completed their referrals (e.g. specialty consults, lab, imaging).

Outcome

In the process of piloting a web-based dashboard incorporated with the EHR system, which help providers to keep track of  the status of their patients regarding completion of the referrals.

SOAR

How might we maximize providers' confidence about ownership of a SOAR patient's care?

Challenge

SOAR (Supporting Older Adults at Risk) is a project aiming to coordinate and provide geriatric-focused, home-based care for older patients recently hospitalized. During the course of the pilot, nearly a quarter of the patients have at some point visited the ED, with a large percentage re-admitted. It is unknown why they still return and get admitted, though having the resources SOAR already provides.

Outcome

Identified the problem to be in providers' confidence about ownership of a patient's care and proposed two solutions leveraging human-human interaction.
